healthUp is a new model of care that combines technology and high-quality primary care focusing on clinical and behavioral science to prevent and control chronic diseases.
As part of it, we have built a Primary Care Clinical product that our members can trust with their health. We want to make it easy for members to access high-quality, evidence-based care, addressing chronic conditions at the root, reducing complications, and preventing costly hospitalizations.
We are re-designing care pathways putting the patient at the center, and creating a new foundation for chronic condition management. Many chronic conditions can be managed from home through remote monitoring, care coordination, home lab testing, and doctor supervision.
